The Add Clinic dialog box displays as follows:
| · | Type the name of your clinic in the Clinic Name field. (*Mandatory field) |
| · | Type the clinic address in the Address field.(*Mandatory field) |
| · | Type the nearest landmark name in the Landmark field.(Not mandatory) |
| · | Click the arrow in the Country field to select the country from the drop-down list.(*Mandatory field) |
| · | Type the name of the state in which your clinic is located in the State field.(*Mandatory field) |
| · | Type the name of the city in which your clinic is located in the City field.(*Mandatory field) |
| · | Type the name of the locality in which your clinic is located in the Locality field.( Not mandatory) |
| · | Type the pin code of your clinic in the PinCode field.(*Mandatory field) |
| · | Type the clinic contact telephone number in the Clinic Contact No. field.(*Mandatory field) |
| · | Select the SendSMSNotification check box for notifications by SMS to your mobile number given during registration. (*Mandatory field) |
| · | Click the AddClinic button again to add the clinic to your list of clinics on MyClinics page.(*Mandatory field) |
| · | Click the Cancel button if you wish to discard the details of the clinic you were adding.(*Mandatory field) |
